Permission Slips & Medical Release Form

We use this website for students to sign up for events.  On the calendar pages under Middle and High School, there will be information about the nearest event and a place for the student to sign up.  Also listed will be a deadline by which the student must turn in a signed permission slip and any required deposit.  Balances are due on the day of the event.

The permission slip may seem cumbersome, but it provides for us the information we'd need to give your student immediate care in the unlikely event that it's needed.  We used to require a new form for each event.  Now, we only ask that you complete the form once each season (Spring, Summer and Fall). Please keep in mind that submitting a permission slip for the Summer, for example, doesn't register your student for every Summer event; your student still needs to sign up online and pay any required deposit.

Completed forms can be returned by mail (ACAC Student Ministries, 250 E Ohio St, Pgh, PA 15212), by dropping it in any campus dropbox, or by bringing it to the Student Ministries office.  To turn in funds on campus, please use the dropbox in the Student Center on the 1st floor of the Whiteside Building, using the provided deposit envelopes.

No Printer? No Problem. This form can be found in the Student Ministries office or Student Center, both on the 1st floor of Whiteside.
